Time to Update Annual Forms

We are asking parents to verify/update their student information through the PowerSchool Parent Portal.   The Annual Update forms will be available for returning students from August 1st through September 15th. These forms are located in the Parent Portal under Forms in the left navigation window, and then under the General Forms tab.  The forms included in the update are:

  • Health Information
  • Student Address
  • Student Contacts & Custody
  • Permissions/Agreements
  • Transportation Request Form

ADDRESS CHANGES WITHIN DISTRICT

If your address has changed within Anchor Bay School District, please be sure to update the Student Address form and provide us with two (2) proofs of residency and your photo I.D. for the new address.  Proofs of Residency can be uploaded through the form or they can be turned into your student’s school on or after August 16th.

MOVED OUT OF THE DISTRICT

If you have moved out of the Anchor Bay School District and want your student to continue enrollment through Anchor Bay Schools, please update the Student Address form and provide us with two (2) proofs of residency and your photo I.D. for the new address.   Proofs of Residency can be uploaded through the form or they can be turned into your student’s school on or after August 16th.  You will also need to complete the 2024-2025 Out of District Schools of Choice Application which is located under Forms in the left navigation window and then under the Enrollment tab in the Parent Portal.   The Schools of Choice application window ends on August 30, 2024.  NOTE:  If you have completed the Schools of Choice Application and been approved in a previous year, you do not need to complete the form again unless your student has left the district and now wants to return.
